I've enclosed the quark file as version 5 and 6:
80x80x6 -> Version 6
80x80x6-ver5 -> Version 5

I've also enclosed a 'failsafe' folder, in which I've output the above as a graphic and placed back in the quark layout, thereby removing any dependancys on fonts etc.
this is in the 'failsafe' folder.
